Documentation

Enclosed are the documentation forms for ultrasounds performed in the deparatment.

The "Attending Documentation" form is for recording patient images in the patient's medical record. The attending documentation should include a labeled form, interpretation, credentialed provider's signature, and attached images. All images should be labeled with the patient's name and medical record number. The number of images recorded is variable and dependent on the type of exam. You should include the minimum number of images to adequately demonstrate pathology or lack of pathology.

The "Resident Documentation" forms are for recording images obtained for ultrasound training. Practice scans can be performed without need for a confirmitory study on any patient less than 40 years of age as long as it does not involve an organ system relevant to a patient's complaint. Practice scans can also be performed on any patient scanned who has a confirmatory study or any patient scanned with a credentialed provider.
